While using PSV’s websites, we may ask you to provide us with certain personally identifiable information that can be used to contact or identify you. Personally identifiable information may include, but is not limited to:

Rest assured, when you submit information on a contact us form, or through email to PSV, we only use your personal information for the use it was intended or for any additional uses specifically provided on this site.

When you visit PSV’s website(s), PSV automatically collects information about how you use its website(s), such as what was searched for and viewed and what devices you use to access the website(s), including computers, tablets, and mobile phones. PSV only shares sensitive data that contains protected health information (PHI)/personally identifiable information (PII) with business associates who are engaged by PSV to assist with carrying out our health care activities and functions and agree to provide protections as required by HIPAA. Any data shared with third parties who do not have a Business Associate Agreement (BAA) with PSV is de-identified (anonymized) before being shared. At times, we may find it necessary to use personally identifiable information as a means to keep you informed of possible services that may be available to you from www.PSVCare.org.
