Must be requested at time of visit. If the refill is needed outside of a regular visit:
- Ask your pharmacy to fax a refill request to 703-839-8764 prior to your prescription running out or expiring.
- Patients need to be seen at least once a year for refills.
- Please allow 5-10 business days to process your request.
For Forms
If you need your provider to complete forms outside of your scheduled appointment, please allow 7-10 business days for completion.
